The 4 Best Exercises to Do After a Total Knee Replacement

After total knee replacement, a structured physiotherapy rehabilitation program helps the knee heal, regain strength and restore flexibility. Here are four of the most valuable exercises commonly used in early knee replacement recovery.

Getting started safely

These exercises are typically introduced progressively following knee replacement surgery, guided by your physiotherapist based on your individual healing and surgical approach — always follow your specific post-surgical program rather than starting exercises independently.

1. Straight leg raises

Builds quadriceps strength without bending the new joint, which is especially valuable in the early post-surgical period. Lying down, tighten the thigh muscle with the knee straight and lift the leg to about the height of the opposite knee, then lower slowly.

2. Knee flexion and extension (heel slides)

Sitting on the edge of a chair with feet flat on the floor, slowly bend the knee by sliding the foot back, then straighten the leg again. This exercise gradually improves range of motion, which is one of the key early milestones after knee replacement.

3. Quad sets

Simple but important — tightening the thigh muscle while the leg is straight, without moving the joint. This helps activate the quadriceps early, even before more dynamic exercises are appropriate.

4. Assisted walking practice

Structured walking practice, initially with a frame or crutches as advised by your surgical team, is one of the most important "exercises" in early recovery — helping restore a normal walking pattern and building tolerance for daily activity.

Progressing beyond the early stage

As healing progresses, your physiotherapist will introduce more advanced strengthening and balance exercises, tailored to your recovery and goals. Consistency with your prescribed exercises between physiotherapy sessions is one of the biggest factors in a smooth recovery.
