Understanding and Managing Chronic Low Back Pain

By Aaron Babb · 12 January 2024

Chronic low back pain affects millions of people and can significantly impact quality of life, work and the activities you enjoy. Unlike acute back pain, which typically resolves within weeks, chronic pain — lasting more than three months — requires a different, more comprehensive approach.

Why chronic back pain is different

Persistent pain doesn't always mean ongoing tissue damage. Over time, the nervous system can become more sensitive to pain signals, meaning pain can persist even as the original injury heals. This is why effective management of chronic back pain looks different from managing a fresh injury — it needs to address both the physical and the broader contributing factors.

Common contributing factors

Deconditioning and reduced activity (often from avoiding movement out of fear of pain), muscle weakness and stiffness that develops over time, stress and sleep disruption, and unhelpful beliefs about pain (such as assuming any pain means damage) can all contribute to chronic back pain persisting.

How physiotherapy helps

  • Individualised assessment — understanding your specific pattern of pain, movement and contributing factors
  • Graded activity and exercise — rebuilding strength, flexibility and confidence in movement, at a pace that doesn't provoke flare-ups
  • Education — understanding pain science helps reduce fear and supports a more active recovery
  • Manual therapy — used alongside active treatment to support comfort and movement
  • Goal-focused rehabilitation — working toward the specific activities and goals that matter to you, whether that's returning to work, sport or simply moving more freely

Managing flare-ups

Chronic pain often involves good days and bad days. Learning to manage flare-ups without losing progress — rather than stopping activity altogether — is a key part of long-term management.

The evidence-based approach

Modern chronic pain management focuses on staying active, building capacity gradually, and addressing the whole picture rather than searching for a single "fix." This approach consistently produces better long-term outcomes than rest alone.
