Understanding your rotator cuff injury

The rotator cuff is a group of four muscles and their tendons that stabilise the shoulder joint. Injuries can range from mild irritation and inflammation through to partial or complete tears, and the right treatment approach depends heavily on which pattern you're dealing with.

Our approach to assessment

A thorough assessment — including understanding how the injury occurred, your specific symptoms, and hands-on testing — helps determine the likely nature and severity of your rotator cuff injury, guiding whether imaging is needed and what treatment approach is most appropriate.

Conservative management first

For many rotator cuff injuries, including a good proportion of partial tears, a well-structured physiotherapy program is the first and often most effective line of treatment. This typically includes:

  • Pain and inflammation management in the early stage
  • Restoring range of motion without aggravating the injury
  • Progressive strengthening of the rotator cuff and surrounding shoulder muscles, which is central to both recovery and preventing further injury
  • Movement retraining to reduce ongoing strain on the injured tissue
  • Gradual return to normal activity, sport or work

When surgery may be considered

For significant full-thickness tears, particularly in younger or highly active patients, or when conservative management hasn't produced adequate improvement, surgical repair may be discussed. Even then, physiotherapy plays a central role in both pre-surgical preparation and post-surgical rehabilitation.
